Why executive health is its own category

An executive physical is not a longer annual checkup. It is a different category of medicine — built for operators whose decisions, schedules, and load profiles are not the average patient's.

Pilots, founders, surgeons, and principals share an environment: chronic time pressure, irregular sleep, high cognitive output, and the assumption that the body will keep up. Standard preventive care was not designed for that load.

02 · Diagnostics

Beyond the full body checkup

A standard full body checkup reports normal ranges. A modern executive health workup reports operational risk — and acts on it.

The minimum viable stack: advanced lipid panel (ApoB, Lp(a)), insulin and HOMA-IR, hsCRP, HbA1c with CGM context, hormonal panel, ferritin and iron studies, full thyroid, vitamin D, homocysteine, omega-3 index, and DEXA for body composition and bone density.

VO2 max, resting HRV, and grip strength are treated as longevity biomarkers — not gym metrics.

Cardiometabolic risk in high-output operators

Cardiovascular disease is the leading cause of avoidable career interruption in high-performance professionals. The risk does not announce itself — it accumulates silently across years of stress, travel, and irregular fueling.

Operational cardiology uses ApoB, Lp(a), advanced lipid subfractions, blood pressure trends, and coronary calcium scoring to identify risk a decade or more before standard panels would.

Intervention at this stage is straightforward. Reversal after a clinical event is not.

Metabolic health is the base layer

Most performance, weight, and energy complaints in executives map back to metabolic dysfunction — insulin resistance, glucose volatility, visceral fat, and mitochondrial fatigue.

Continuous glucose monitoring, fasting insulin, and a basic body composition scan tell you more about long-term healthspan than a 50-marker blood panel without context.

Cognitive performance and brain longevity

Executive cognition runs on sleep architecture, glucose stability, cardiovascular health, and inflammatory load. Decline in any one of those quietly degrades judgment, recall, and decision speed years before it is named.
